SIOUX CHAPTER 2008/2009 WELLNESS INCENTIVE AWARD PROGRAM
1. The goal is to reach 85 points. You may begin accumulating points on Saturday, October 18, 2008.
2. One can earn only one point per day. If you earn more than one point per day, it cannot be saved and used at a future time.
3. Points are earned by completing an activity. (See the activity sheet.) Many of the fitness/physical activities have minimum time requirements. The time requirements do not need to be done all at one time. It can be spread over an entire day.
4. Points must come from at least two of the six main wellness areas. (See activity sheet.) However, a minimum of 60 points must come from the fitness/physical area. Twenty five points must come from any of the other areas of wellness.
5. Please keep your own record on the wellness log provided. If you complete a physical activity use a P or X in the Physical column on your log and a O or X in the Other column for completing an activity in the other areas of Wellness.
6. Keep accumulating points after you reach your 85 points. Those with the most points will have a chance to participate in a drawing at the end of the year. You will also be accumulating points for your team. If you need additional Wellness logs, please notify John Hegg at 339-2582 or johnhegg@sio.midco.net
7. All completed wellness logs need to be sent to John Hegg at 2612 Arcadia Road, Sioux Falls, SD 57105 by Friday, April 17, 2009. A “celebration of wellness" will be held May 21, 2009.
